安徽淮北地区超广谱β-内酰胺酶细菌表型检测的底物选择

摘要 目的了解淮北地区产超广谱β-内酰胺酶细菌的表型分布及检测的最佳底物选择。方法用1999年NCCLS推荐的标准,采用纸片扩散法从147株大肠埃希菌和肺炎克雷伯菌中筛选出可疑产ESBLs细菌,并用确证试验加以确认。结果116株大肠埃希菌和31株肺炎克雷伯菌中共检出69株产ESBLs细菌(46.9%),将初筛得到的可疑菌株用纸片扩散法进行确证试验,头孢噻肟和头孢噻肟/克拉维酸测定,检出69株产ESBLs细菌,而用头孢他啶和头孢他啶/克拉维酸检测ESBLs阳性菌为32株。结论淮北地区进行ESBLs细菌检测时,应选择头孢噻肟和头孢曲松作为初筛试验的底物,选择头孢噻肟和头孢噻肟/克拉维酸测定作为确证试验的底物。 Objective To investigate the phenotype distribution and perfect substrates of strains producing extended-spectrum β-lactamases (ESBLs) in Huaibei area.Methods Double paper diffusion screening test and phenotypic confirmatory test from NCCLs (National committee for clinical laboratory standards)were performed to detect the 147 strains of K.pneumoniac and E.coli of ESBLs.Results Sixty-nine strains of ESBLs were detected from 116 strains of E.coli and 31 strains of K.pneumoniac (46.9%).In double paper diffusion test, 69 strains of ESBLs were certified from the suspected strains screened by Cefotaxime and Cefotaxime/Clavulanate,and 32 strains of ESBLs certified by Ceftazidime and Ceftazidime/Clavulanate.Conclusion Cefotaxime and Ceftriaxone can be substrates for the screening test and Cefotaxime and Cefotaxime/Clavulanate can be the substrates for the certified test in Huaibei area.
