腹腔镜直肠癌根治术近期疗效分析

Clinical Study of Laparoscopic Operation for Rectal Carcinoma

目的:探讨经腹腔镜行直肠癌根治术的近期疗效。方法:30例经腹腔镜行直肠癌根治术,与同期行传统开腹手术20例作对照,比较2组患者手术时间、术中出血量、术后近期情况及随访的局部复发率和远处转移率。结果:腹腔镜组和传统手术组术中出血量分别为(160.9±70.3)mL和(300.2±50.5)mL(P<0.05);腹腔镜组术后肛门首次排气时间、下床活动时间均早于传统手术组(P<0.05);术后病理学检查和术后随访的疗效比较,差异无显著性(P>0.05)。结论:经腹腔镜行直肠癌根治术与传统开腹手术比较,近期效果满意且有其独特优点。

Objective:To study the clinical outcomes of laparoscopic resection of rectal carcinomas.Methods: laparoscopic and traditional operation groups during the periods of 2001.12-2004.6 were contrasted in the way of the length of intestinal segment resected,the size of tumor, the number of lymph node excised and local recurrence and distant metastasis rate after the operation.Results:No significant difference was found in tumor size between the 2 groups. The total number of lymph nodes excised: Laparoscopic group12.94±7.38. Traditional operation group12.26±7.97. The distance from the lower end of resection to the rectum(3.85±2.33)cm、(3.57±2.18)cm,respectively. No difference was observed between 2 groups in the local recurrence rate and distant metastasis. Conclusion: Laparoscopic rectal surgery has certain advantages over conventional open surgery,the short-term outcomes is satisfying.
