摘要
目的探讨B超在神经外科手术中实时定位的应用价值。方法常规开颅后在硬膜外应用B超,结合术前影像学资料,进行病灶的实时定位,帮助选择皮层切口和手术路径,并用B超判定病灶切除程度。结果33例中明确定位28例,成功率84.8%,定位准确的28例中,病灶全切21例,大部切除7例,2例术后神经功能障碍加重,无严重手术并发症,无围手术期死亡。结论术中B超对皮层下的囊性和密度较高、边界清楚的实质性病灶显示清楚,定位准确,可帮助术中确定皮层切口和手术路径,减少神经功能障碍的发生。
Objective To introduce the use of B-mode ultrasound in neurosurgical operation. Methods Combined with preoperative image data,we used B-mode ultrasound probe to find brain lesion after craniotomy,analysis of resection degree was performed by ultrasound. Results Total definite orientation in 28 of the 33 cases,successful rate 84.8% Total lesion resection was achieved in 21of the 28 cases, subtotal resection in 7 cases.Postoperative neurologica functions were improved or unchanged in 31cases,worsen in 2 cases. Conclusion We can get the real-time position and image of brain lesion with intraoperative ultrasound. B-mode ultrasound is helpful for total resection of lesion and decrease the side effect injury.
