摘要
目的:提高对肺上皮样血管内皮瘤影像表现的认识。方法:报告2例经病理证实的肺上皮样血管内皮瘤的影像所见,并作文献复习。结果:2例年轻女性,反复咳嗽,咯血痰。胸片和CT显示两肺多发小结节影,直径在1～12mm范围,大部分结节直径在2～4mm之间,部分结节见胸膜牵拉征;部分支气管血管束不均匀增粗。1例合并肝内多发大小不一低密度灶,大小约10～50mm,增强扫描病灶呈不均匀环形强化。结论:肺内弥漫性小结节并有支气管血管束增粗是肺上皮样血管内皮瘤的影像特点。
Objective: To enhance the recognition of X-ray appearance of Pulmonary epithelioid hemangioendothelioma (PEH). Methods: Report of X-ray appearance of two cases of pathologically proven PEH and review the literature. Results: The clinical characteristics of the two cases were young female, presenting with chronic persistent cough and hemoptysis. Chest roentgenography and CT showed multiple nodules with diameter ranging from 1mm to 12mm, most of them in the range of 2mm to 4mm. Pleural dragging sign was found in some nodules. Thickening of broncho-vascular bundle was observed in some area. Multiple low density shadows in a variety of diameter ranging from 10mm to 50 mm were found in one case. Irregular ring-shaped enhancement was observed in enhancement scanning. Conclusions: The characteristics of pulmonary epithelioid hemangioendothelioma was diffuse nodules and thickening of broncho-vascular bundle.
