吸收不良综合征患者十二指肠纹缘上皮细胞微绒毛超微结构与图像分析

Study of Ultrastructural Changes and Image Analysis of Duodenal Striated Epithelial Microvilli of the Patients With Malabsorption Syndrome

对纤维胃镜咬取的18例消化不良患者的十二指肠纹缘上皮微绒毛,进行了超微结构观察和图像分析研究。结果显示,7例吸收不良综合征患者的微绒毛平均长度、密度及表面积等均较11例非吸收不良综合征患者(胃炎5例、结肠炎6例)的微绒毛明显短和小,差异非常显著(P<0.01)。部分病例还显示有微绒毛长短不一、方向紊乱及花束样粘连等改变。病变轻重与临床表现基本相符,治疗好转者微绒毛病变改善。提示本病患者十二指肠微绒毛病变可基本代表小肠病变概貌。本文提出的吸收不良综合征临床诊断标准,亦经本资料证实有实用价值。

The ultrastructural changes and image analysis of duodenal striated epithelial mi-crovilli of 18 patients with dyspeptic were studied. The specimens were obtained by fibrogas-troscop biopsy. The result indicated that the e-varage length, density and surface areas of mi-crovilli in 7 patients with malabsorption syndrome were all markedly shorter and smaller than those in 11 patients with chronic gastritis or colitis (P<0. 01). In some cases of the 7 patients, the microvilli also showed no-uniform disorder of direction or flower-benchoid adhesin-lect. The degree of the microvillial abnormal i-ties conformed basally with the clinical represents. The pathological changes reduced with the heading of the disease . It sugest that, the basic pathological changes of the small intestinal mu-cosa may be represented by that of the duodenal mucosa of the patient with malabsorption syndrome. On the other hand, we proposed the clinical diagnostic criteria of malabsorption syndrome.
