何杰金病的免疫组织化学研究

Immunohistochemical Study on Hodgkin's Disease

收集有完整临床和病理资料的何杰金病30例,用LCA、L_(26)、UCHL_1、Mac_(387)、EMA、BerH_2和Vimentin作免疫组化染色。结果显示何杰金细胞(HC)、Reed-Sternberg cell(R-SC)的表达如下:6例UCHL_1染色阳性,17例BerH_2染色阳性,其阳性率与组织学分型无相关性,同时显示BerH_2对HC和R-SC的检出是很有价值的;淋巴细胞为主型(LP)中2例LCA阳性,5例L_(26)阳性;而其他各型均为阴性,只有LP中1例其HC和R-SC EMA染色为阳性,而Mac_(387)和Vimentin对所有病例染色均为阴性。淋巴细胞的染色则表明不存在单一B或T细胞的免疫表达,只是以何种细胞为主而已,且阳性淋巴细胞常在HC和R-SC的周围呈卫星状分布。综合HC、R-SC和淋巴细胞的染色,有5例同时显示L_(26)阳性反应的均为LP,6例同时显示UCHL_1阳性反应的各型均有,提示HC、R-SC和淋巴细胞之间存在某种内在联系。本结果提示免疫组织化学对HD的诊断以及判断肿瘤细胞的组织学来源有一定的价值。

30 cases of Hodgkin's disease were analyzed immunohistochemically. Among them, there were LP 10 cases, MC 10 cases, NS 5 cases and LD 5 cases. A panel of monoclonal and polyclonal antibodies composed of LCA, L26, UCHL1, Mac387, BerH2, EMA and vimentin was used Hodgkin's cell (HC) and Reed-stern-berg cell (R-SC) of 6 cases were positive for UCHL1, and 17 cases were positive for BerH2 there was no correlation between positive rate and histological type, and showing BerH2 stain is useful in detecting the HC and R-SC. LP were positive for LCA (2/10) and L26(5/10) while other types were constantly negative. Only 1 cases of LP HC and R-SC were positive for EMA. Mac387 and vimentin were constantly negative. These supported a B cell or T cell derivation for HC and R-SC. Lymphocytes in all cases were composed of T and B cell, these cells di-posed arround the HC and R-SC. In 5 cases of LP, HC and R-SC exhibited immunoreactivity for L26, while lymphocytes also showed postive reactivity. In 6 cases of four type HC, R-SC and lymphocytes were positive for UCHL1. These data perhaps will be of great help in understanding relationship between HC, R-SC and lymphocytes.
