类似于急性硬脑膜下血肿的蛛网膜下腔出血

Acute subdural hematoma-like traumatic subarachnoid hemorrhage

目的探讨头颅CT颅骨缘下“新月形”高密度影的特征性改变,提高对类似于急性硬脑膜下血肿(ASDH)的蛛网膜下腔出血的认识。方法从临床表现、CT影像、治疗方法及效果、预后等方面对28例类似于ASDH的外伤性蛛网膜下腔出血(tSAH)患者进行了回顾性分析。结果28例tSAH均表现为骨缘下“新月形”高密度影,手术5例,3例治愈;非手术治疗23例,20例治愈。结论tSAH可表现为类似于ASDH的骨缘下“新月形”高密度影,非手术治疗有较好的效果。

Objective To investigate the diagnosis of CT manifestation of 'crescent/high-intens signals under skull and clear the wrong conception of equaling them to acute subdural hematoma(ASDH).Methods Clinical studies of 28 cases with traumatic subarachnoid hemorrhage(tSAH) were reviewed retrospectively,based on clinical presentation,CT scans,therapectical choice,effect and prognosis.Results CT manifestations of all cases were 'crescent' high-intens signals under skull.Effects of non-operative treatment were better than those of operative treatment.Conclusion CT manifestations of tSAH may present 'crescent'high-intens signals under skull similar to ASDH,and differential diagnosis must be taken.
