超声检查在腹腔镜子宫肌瘤剔除术中的应用价值

Clinical Value of B-ultrasonography in the Laparoscopic Myomectomy

目的探讨超声检查在电视腹腔镜子宫肌瘤剔除术(LM)前中后的价值及临床意义。方法对98例子宫肌瘤患者行LM用超声进行常规检查,并与手术、病理结果对照。结果本组98例超声诊断与手术病理符合95例,符合率96.94%,LM成功90例,占91.94%,中转开腹8例,并发症5例,漏误诊3例,有1例在LM中发现为子宫肉瘤,2例为子宫腺肌瘤。结论超声检查子宫肌瘤为临床提供了良好手术前选择的依据,同时对手术难度的预测、术中的定位、术后并发症的发现及动态观察疗效等方面有较大的实用价值,是LM前中后首选和理想的影像诊断方法。

Objective To investigate the clinical v alue of ultrasonography in the laparoscopic myomectomy (LM). Methods The comparison and analysis between the B-ultras onograms and the pathologic changes were made in 98 cases of uterine leiomyoma s ubjected to LM. Results The B-ultrasonographic diagnosis of 95 cases con formed by pathologic changes (accordance rate of 96.94%).LMs in 90 cases were s uccessful(91.94%);8 cases were converted to open laparotomy procedures;5 cases emerged post-operative complications;3 cases were misdiagnosed pre-operatively ;1 case turned out to be uterine leiomyosarcoma after LM;2 cases were confirmed to be uterine adenomyoma. Conclusions Ultrasonography can provide scientific and re liable basis for treatment of leiomyoma pre-operatively,and it plays an impo rtant role in predicting the risk of operation and intra-operative localization ,discovering post-operative complications and evaluating curative effect dynami cally.It may be one of the optimal diagnostic imaging methods in LM.
