COPD与支气管哮喘合并COPD在肺通气功能、气道激发试验及疗效的临床观察

To observation the COPD and the Bronchial asthma combined with COPD At pulmo aeration function, air passage provocation test and curative effect in clinic

目的:通过比较支气管哮喘合并COPD和COPD在肺通气功能、气道激发试验及疗效的特征,以利鉴别诊断和治疗。方法:本文对45例支气管哮喘合并COPD,42例COPD患者进行肺通气功能、气道激发试验及疗效进行对照观察。结果:1.支气管哮喘合并COPD组小气道气流受阻明显较COPD组严重(P<0.001),气道阻力明显增高(P<0001)。2.支气管哮喘合并COPD组100%显示气道激发试验阳性,而COPD组仅为13.5%,COPD组阳性者所需激发剂(组胺)浓度为支气管哮喘合并COPD的350倍,且两者间较少重叠交叉。3.在临床常规治疗的疗效上看支气管哮喘合并COPD组优于COPD组(P<0.001)。结论:支气管哮喘合并COPD和COPD虽然在临床上很相似,但从肺通气功能、气道激发试验及疗效等特征均有显著的差异。

Objictive:To compare characteristic that the COPD and the Bronchial asthma combined with COPD at pulmo aeration function, air passage provocation test and curative affect in clinic, so that benefit the ant diastole and treatment. Methods: Use the measure of clinic control investigation test to compare two group patients' conditions in pulmo aeration function test, air passage provocation test and curative effect, and statistical analysis. Results: 1.The group that Bronchial asthma combined with COPD take place air current suffocate at small air passage more than other group (P<0.001). The air passage resistance raised more than other group (P<0.001). 2. 100% cases of bronchial asthma combined with COPD group indicate positive chronotropism in air passage provocation test, but there are 13.5% cases in COPD group, the positive chronotropism of cases in COPD group require concentration ratio of booster (beta-imidazolylethylamine) more than 350 times, and few overlapping and decussation at concentration ratio between two group. 3. At the curative effect in clinic, the group that Bronchial asthma combined with COPD overmatch the other group (P<0.01). Conclusions: Although Bronchial asthma combined with COPD was very similitude in clinic, but from the pulmo aeration function test, air passage provocation test and curative effect t compare, there are a lot of character have distinct difference in statistical analysis.
