微卡辅助治疗老年初治菌阳肺结核疗效对比观察

Effect of Vaccae as Auxiliary Treatment for Sputum Positive Pulmonary Tuberculosis among Old People

目的观察和评价微卡辅助治疗老年初治菌阳肺结核的疗效。方法将120例老年初治菌阳肺结核病人随机分为治疗组和对照组各60例。对照组采用2HRZE/4HR方案治疗。治疗组采用上述方案治疗的同时,加用微卡22.5μg,臀部肌内注射。每周1次,疗程2个月。然后观察痰菌阴转,肺部病灶吸收、空洞闭合等情况。结果痰菌阴转情况:1个月后治疗组46例,对照组32例,两组比较,差异有非常显著性(P<0·01);2个月后治疗组55例,对照组43例,两组比较,差异有非常显著性(P<0·01);3个月后治疗组60例,对照组58例,两组比较,差异无显著性。病变吸收情况:2、4个月及疗程结束后显效病例,治疗组分别为27、48、52例,对照组分别为14、32、40例,两组比较,差异均有非常显著性(P<0·01)。空洞闭合情况:2、4个月及疗程结束后空洞闭合病例,治疗组分别为11、19、21例,对照组分别为4、13、19例,两组比较,差异均有显著性(P<0·05)。结论微卡辅助治疗老年初治菌阳肺结核有显著性疗效。

Objective To observe the effect of Vaccae as auxiliary treatment for sputum positive pulmonary tuberculosis among old people. [Methods] 120 patients were divided into two groups, namely the case group and the control group. The control group were treated with 2HRZE/4 HR , the case group were treated with 2HRZE/4 HR combined with injection of Vaccae of 22.5 ug, once a week for a period of two month. Observation was made on the sputum negative conversion, focus absorption and cavity closure. [Results] The sputum negative conversion is as follows: 46 cases in the case group and 32 cases in the control group after 1 month, the difference was significant (P<0.01); 55 cases in the case group and 43 cases in the control group after 2 months, the difference was also significant (P<0.01); 60 cases in the case group and 58 cases in the control group after 3 months, the difference was not significant. The focus absorption was as follows: 27, 48 and 52 cases in the case group and 14, 32 and 40 cases in the control group after 2 moths, 4 months and after treatment respectively (P<0.01). Cavity closure is as follows: 11, 19 and 21 cases in the case group and 4,13 and 19 cases in the control group after 2 month, 4 months and after treatment respectively, the difference was significant (P<0.05). [Conclusion]Using Vaccae as auxiliary treatment for sputum positive pulmonary tuberculosis among old people is of significant effect.
