应用颈前小切口无气腔室内镜下甲状腺手术43例的临床体会

Experience in the micro-invasive video-assisted thyroidectomy

目的探讨小切口无气工作腔室内镜下甲状腺手术可行性及效果。方法颈前单一2cm小切口径路,借助外牵吊技术建立无气工作腔室。以高频超声刀剪作为主操作器械,辅以特选吸引剥离器等,对43例甲状腺腺瘤和结节性甲状腺肿病人实施内窥镜下手术。结果手术均获成功。无1例因出血失控或术后创口内出血而转为开放手术。结论颈前小切口无气腔室内镜下甲状腺手术是安全可靠的方法。既达到微创的目的,又具有一定的美容效果。

Objective To study the feasibility and effect of the micro-invasive video-assisted thyroidectomy. Methods A total of 43 cases of adenomas and nodular goiters consecutively received the endoscopic operation. A gasless working space was created with conventional retractor through a 2-cm small incision at anterior neck. The procedure was designed to use ultrasonic scalpels and scissors as major instruments, assisted with suction-detacher or other tools. Results All the operations were successful, and none was interrupted or converted to open surgery due to uncontrollable bleeding or severe postoperative hematoma. Conclusion The micro-invasive video-assisted thyrodectomy is safe, feasible and minimally invasive.
