白内障摘出治疗闭角型青光眼的临床观察

Treatment of angle-closure glaucoma by extraction of cataract

目的评价单纯白内障摘出加后房型人工晶状体植入术治疗闭角型青光眼合并白内障的疗效。方法回顾性研究2001年8月～2003年6月收治的资料完整的闭角型青光眼合并白内障17例(20眼),单纯施行超声乳化白内障摘出或小切口白内障囊外摘出联合后房型人工晶状体植入术,术后随访3～18月。结果所有患者术中、术后均没有出现严重的并发症,术后视力均较术前提高,18眼术后眼压正常,2眼加用抗青光眼滴眼液控制正常,术后平均眼压(16.75±3.10)mmHg与术前眼压(25.31±11.60)mmHg相比,差异有显著统计学意义(配对t检验P<O.05),术后平均前房深度(2.56±0.68)mm与术前(1.31±0.62)mm相比差异有统计学意义(配对t检验P<0.05),术后所有病例前房角均重新开放或部分开放增宽。结论单纯白内障摘出人工晶状体植入是治疗闭角型青光眼合并白内障的一种有效选择。

Objective To evaluate the effect of angle-closure glaucoma with cataract treated by extraction of cataract and posterior chamber lens implantation. Methods Angle-closure glaucoma with cataract treated by phacoemulsification or extraction of cataract and posterior chamber lens implantation in 20 eyes(17 cases) were studied retrospectively from Aug.2001 to Jun.2003,and followed up 3-18 months postoperatively. Results All cases hadn't serious complication and vision improved in 18 eyes.lntraocular pressure was controlled to normal but 2 eyes needed antiglaucoma agent. The intraocular pressure changed from 25.31±1.60 mmHg preoperatively to 16.75±3.10 mmHg postoperatively (P<0.05 t-test).The depth of anterior chamber changed from 1.31±0.62 mm to 2.25±0.68 mm(P<0.05 t-test).After operation all cases'chamber angle reopened or some part of chamber angle became wide. Conclusion To treat angle-closure glaucoma with cataract by extraction cataract and lens implantation was an effective choice
