摘要
观察研究抗幽门螺杆菌抗体牛奶(HPU-牛奶)辅助治疗胃内幽门螺杆菌(H.Pylori)感染的临床意义。采用14C-尿素呼气试验(14C-呼气试验)对36例受检者进行呼气积分值检测。其中18例阳性者根据积分情况分为四组(I组:(25-100)脉冲/s;Ⅱ组:(101-200)脉冲/s;Ⅲ组:(201-300)脉冲/s;Ⅳ组:>300脉冲/ s),并各自服用HPU-牛奶16周,分别观察各组受试者服奶后1、2、4、8、16周时的14C-呼气试验积分值情况。结果36例14C-呼气试验受检者中,18例阳性者和18例阴性者积分均值分别为170.33脉冲/s和-2.22脉冲/s,二者比较差别显著(P<0.01)。四组14C-呼气试验阳性者服用HPU-牛奶前、后积分均值比较显示:I 组服奶前积分均值68.25脉冲/s,与服奶后2周时积分均值39.0脉冲/s比较差别显著(P<0.01),与其他各周积分均值比较均无差别(P>0.05);Ⅱ组服奶前积分均值137.78脉冲/s,与服奶后各周积分均值比较有显著差别(P<0.001);Ⅲ组服奶前积分均值266.33脉冲/s,与服奶后各周积分均值比较均有显著差别(P<O.01);Ⅳ组服奶前积分均值377.0脉冲/s,除与服奶后8周时积分均值93.0脉冲/s比较差别显著外(P<0.001),与其他各组积分均值比较均无差别(P>0.05)。结论是无创的14C-呼气试验是高敏感性和高特异性的H.Pylori感染现症感染即时诊断方法,其积分值检测可量化受检者H.Pylori感染情况,便于临床疗效的监测。HPU-牛奶对于14C-呼气试验检测的低积分值H.Pylori感染者有一定的辅助治疗作用。
The clinical significance of the Helicobacter pylori infection treated by the milk with anti Helicobacter pylori antibody( HPU-milk) is intended to study. 36 subjects through 14C-urea breath test (14CUBT) to examine Helicobacter pylori (H. Pylori) infection are tested. The tests of 18 subjectes were positive. Then they were divided 4 groups (group Ⅰ:(25-100) pulse per second; group Ⅱ :(101-200) pulse per second; group Ⅲ:(201-300) pulse per second; group Ⅳ:>301 pulse per second) and take HPU-milk for 16 weeks. The four groups are investigated respective at 1,2,4,8,16 weeks after taking HPU milk, There was significant difference (P<0. 01) between 18 participants whose tests were positive (170. 33 pulse per second) and 18 participants whose tests were negetive (-2. 22 pulse per second). The results are compared before taking HPU-milk with after taking HPU-milk and found that rn group Ⅰ the result was 68. 25 pulse per second before taking HPU-milk compared with 39. 0 pulse per second after taking HPU-milk for two weeks and they has significant difference (P<0. 01) , but other groups has no difference with the results before taking HPU-milk (P>0. 05). In group Ⅱ the difference is significant between the test before taking HPU-milk(137. 78 pulse per second) compared with per tests after taking HPU milk (P<0. 01). The results of group Ⅲ is similar to group Ⅱ . In group Ⅳ the resulis is s(?) togroup Ⅰ. It is conclused that 14 C-UBT is a noninvasive measure with high sensitivity and high specifity to determine whether the H. Pylori is infected. HPU-milk has an important role in the therapy of H. Pylori infection.
