鼻内窥镜下经额鼻联合径路侵犯前颅底嗅神经母细胞瘤切除术

Esthesioneuroblastoma surgery encroached on anterior skull base by transnasal forhead approach with endoscopes

目的探讨鼻内窥镜下经额鼻联合径路侵犯前颅底嗅神经母细胞瘤(esthesioneuroblastoma,ENB)切除术的可行性和适应证。方法对2003年1月￣2004年5月收治的鼻内窥镜下经额鼻联合径路侵犯前颅底ENB切除术6例患者的临床资料、手术径路及方法、预后进行回顾性分析。结果6例手术均获成功,1例因术后未加放疗半年后复发,再行同一径路手术,术后加做放疗70Gy,观察6个月无复发。结论鼻内窥镜下经额鼻联合径路侵犯前颅底嗅神经母细胞瘤切除术疗效肯定,但需严格掌握适应证,强调综合治疗。

[Objective] To explore the feasibility and indication of esthesioneuroblastoma surgery encroached on anterior skull base by transnasal forhead and nasal approach with endoscopes. [Methods] Between 2003 and 2004, six patients with ENB encroached on anterior skull base had been operated by transnasal forhead and nasal approach with endoscopes. Their clinical datas, approachs and methods of surgery,and prognosis were reviewed and analyzed. [Results] Six patients had succeed, but one had recurred after six months without radiotherapy. Similar surgery had been done again and had radiotherapy, the patient hadn′t recurred after six months. [Conclusion] Esthesioneuroblastoma surgery encroached on anterior skull base by transnasal forhead and nasal approach with endoscopes is effective, but we should master indication strictly and emphasis on synthetic therary.
