急性冠脉综合征血清脂联素水平研究

Serum adiponectin concentration in patients withacute coronary Synedrom

目的探讨急性冠脉综合征患者脂联素水平及其与冠状动脉狭窄程度,肿瘤坏死因子-α,瘦素之间的关系。方法46例急性冠脉患者和42例健康对照组常规测量身高、体重,计算体重指数。抽取空腹静脉血检测其血糖、血脂、甲状腺素、肿瘤坏死因子-α、瘦素、指联素。采用Gensini积分系统评分冠脉狭窄程度。结果急性冠脉综合征患者与健康对照组比较,脂联素水平下降[(3.65±1.68)μg/mL66(7.08±3.24)μg/mL,P<0.01],健康对照组脂联素与体重指数(r=-0.254,P=0.040),总胆固醇(r=-0.261,P=0.046)呈负相关;急性冠脉综合征中脂联素与冠脉狭窄程度(r=-0.349,P=0.014),甘油三酯(r=-0.356,P=0.015),肿瘤坏死因子-α(r=-0.301,P=0.045)三碘甲状腺原氨酸(r=-0.321,P=0.036)呈负相关。多元线性回归提示,在健康对照组中,体重指数是影响脂联素水平的独立因素而在急性冠脉综合征组中,冠脉狭窄程度,肿留坏死因子-α则是影响脂联素水平的独立因素。结论在急性冠脉综合征患者中,脂联素水平下降,其下降水平并与冠脉狭窄程度存在一定相关性。

[objective] To explore the serum concentrations of adioponetin in patients with acute coronary syndrome(ACS), and the relations between adiponectin and severity of coronary artery is stenosis tumor necrosis factor-alpha (TNF-α) and leptin. [Method] Forty six cases with ACS and forty tow health control subjects were taken fasting blood sample for measurements of plasma glucose, plasma lipids, throid hormornes, TNF-α, leptin and adiponectin. [Results] The concerntration of adiponectin in ACS cases were significant lower than thoses in the control group (3.65±1.68) VS (7.08±3.24) μg/mL, P <0.01). Person relation analysis showed that adiponectin levels were negatively and significantly correlated with body-mass index (r =-0.254, P =0.040). Totol cholerterol (r =-0.261, P =0.046) in control group, while adiponectin concentratios was negtively and singnificanty correlated with severity of covonary artery stenosis (r =-0.349, P =0.014), triglyceride (r =-0.356, P=0.015), TNF-α (r =-0.301, P =0.045), and triidothyronine (r =-0.321, P =0.036) in ACS group. Multiple regression analysis showed that body-mass index was independent factor to adiponectin levels in control group. Severity of coronary artery stenosis and TNF-α were adiponectin in dependent factor in ACS group. [Conclusion] The adiponectin concentration was siginificant lower tients with ACS and Here was negative and significant correlation between adiponectin and severity of coronary artery stenosis.
