201例成人急性白血病免疫分型特点

Immunophenotyping of 201 adult patients with acute leukemia

目的探讨成人急性白血病(AL)三色流式细胞术免疫分型的特点。方法采用CD45/SSC双参数散点图设门方法进行三色流式细胞术免疫表型分析。结果156例急性髓细胞系白血病(AML)主要表达CD117(90.38%)、CD13(87.18%)、CD33(71.79%)、CD34(67.74%)、HLA DR(60.90%)。33.98%的AML患者伴有淋系抗原表达,最常见为CD7(24.36%)。B细胞系急性淋巴细胞白血病(ALL)35例(17.4%),主要表达CD19(100.00%)、HLA DR(100.00%)、CD10(74.29%)、CD34(60.00%)、CD20(31.40%)。T细胞系ALL10例(4.98%),主要表达CD7(80.00%)、CD2(60.00%)。24.5%的ALL表达髓系相关抗原,主要是CD13(17.80%)及CD33(11.10%)。结论流式细胞仪三色荧光标记法进行AL免疫分型,对白血病的准确诊断和分型有重要的指导意义。

Objective To evaluate the specificity of the method of three-color flow cytometry applied in immunophenotyping of adult patients with acute leukemia. Methods Immunophenotyping was performed by three-color flow cytometry using CD45/SSC gateing. Results In 156 patients with acute myeloid leukemia (AML), the antigens were mainly CD117(90.38%), CD13(87.18%), CD33(71.79%), CD34(67.74%) and HLA-DR(60.90%). Lymphoid-associated antigen expressed in 33.98% of the AML patients especially CD7(24.36%). In 35 patients with B-lineage acute lymphoblastic leukemia, the antigens were mainly CD19(100.00%), HLA-DR(100.00%), CD10(74.29%), CD34(60.00%) and CD20(31.40%). In 10 patients with T-lineage acute lymphoblastic leuckemia, the antigens were mainly CD7(80.00%) and CD2(60.00%). Myeloid-associated antigen expressed in 24.5% of the ALL especially CD13(17.80%) and CD33(11.10%). Conclusions Three-color flow cytometry is extremely important in diagnosis and immunophenotyping of acute leukemia.
