舌癌颈淋巴结转移的临床分析

Analysis of the regularity of cervical lymph node metastasis in tongue cancer

目的:探讨舌癌颈淋巴结转移的规律。方法:回顾性分析1992年7月～2004年12月间收治的54例舌癌临床资料。结果:本组患者舌癌的转移率是42.59%(23/54);50岁以上患者占75.93%(41/54),其中发生颈淋巴结转移的占51.22%(21/41);受累淋巴结位于颌下区和颈深上区达72.34%(34/47);临床颈淋巴结阴性(cN0)的患者颈淋巴转移率是30.43%(7/23)。结论:舌癌颈淋巴结转移与年龄、肿瘤发生部位密切相关,部分患者可发生早期的隐匿性转移,颌下和颈深上区是舌癌转移的集中部位。

Objective To explore the regularity of cervical lymph node metastasis in tongue cancer. Methods A retrospective research was performed on clinical data of 54 cases of tongue cancer treated with surgery from 1992 to 2004. Results Cervical lymph node metastasis were found in 23 cases. The metastatic rate was 42.59% (23/54). In 54 cases the morbid age of 41 cases were over 50 years old the percentage was 75.92%(41/54). Most primary place was margolinguae. The positive lymph nodes mostly were profund submandibular and lymphonodi cervicales craniales 72.34%(34/47). The metaastatic rate of clinical negative nodes (cN0) was 30.43% (7/23). Conclusion This study revealed that the metastatic rate of tongue cancer was associated with morbid age and tumor primary site. Some cases had occult metastases in early stage. Cervical lymph nodes metastasis mostly occurred in profund submandibular and lymphonodi cervicales profund craniales.
