老年COPD患者呼吸肌力与肺通气功能关系探讨 被引量:15

摘要 目的探讨老年COPD患者的呼吸肌力量及其与肺通气功能的关系。方法测定58名老年COPD患者。男性36例,女性22例;平均(70±3.0)岁的呼吸肌力(最大吸气压MIP、最大呼气压MEP)和肺通气功能(包括用力肺活量FVC、第1秒用力呼气量FEV1、第1秒用力呼气量占用力肺活量百分比FEV1/FVC、最大通气量MVV等),计算出各个对象实测值占其预计值百分比,并计算呼吸肌力与其肺通气功能的相关性。结果老年COPD患者呼吸肌力和肺通气功能均明显低于其预计值,最大呼气压仅占其预计值的(58.2±10.2)%,最大吸气压仅占其预计值的(48.3±9.5)%;FVC%为(73.0±8.8),FEV1%为(57.8±9.7),FEV1/FVC%为(59.0±8.1),MVV%为(48.9±6.4)。经相关分析,呼吸肌力与其肺通气功能呈正相关。结论提示老年COPD患者肺通气功能减退与其呼吸肌力衰弱密切关联,改善呼吸肌状态有利于提高COPD患者的呼吸功能。 [Objective] To study the relationship between respiratory muscle strength and pulmonary ventilation in the old patients with chronic obstructive pulmonary disease(COPD). [Methods] Respiratory muscle strength (MIP, MEP) and pulmonary ventilation (FVC, FEV1, FEV1/FVC, MVV) were detected in old patients with COPD. [Results] MIP, MEP and lung ventilation of COPD were lower than predicted and significantly correlation between MIP,MEP and pulmonary ventilation. [Conclusion] The pulmonary ventilation which decreases in COPD bears a close relationship muscle strength.
