高容量血液滤过对MODS猪肺TNF-α和IL-8 mRNA表达的影响

Effect of high volume hemofiltration on expression of TNF-α and IL-8 mRNA in lungs of a procine multiple organ dysfunction syndrome model

目的:探讨高容量血液滤过(HVHF)后多器官功能障碍综合征(MODS)猪肺组织TNF-α、IL-8mRNA表达水平的变化及意义。方法:雄性家猪25只,随机分为模型组(n=9)、滤过组(n=10)、对照组(n=6),前二组采用二次打击法建立MODS模型,滤过组采用HVHF治疗3d,对照组不作处理。测定各器官功能相关指标,实时荧光定量RT-PCR技术定量法检测肺组织TNF-α、IL-8mRNA表达水平。结果:滤过组滤过后各器官功能有关指标水平均有不同程度的改善,器官衰竭发生率、动物病死率较模型组明显降低(20.0%vs88.9%,20.0%vs77.8%,P<0.05);滤过组TNF-αmRNA、IL-8mRNA表达水平较模型组明显降低[(0.1042±0.0158)vs(0.1235±0.0188),P<0.01;(0.1458+0.0199)vs(0.1991±0.0519),P<0.05]。结论:HVHF早期应用可以降低创伤引起的肺组织TNF-α和IL-8mRNA表达,表达水平与肺损伤的程度和肺衰竭发生率相关;同时可减少肺损伤,减少肺衰竭的发生。

Objective: To investigate the effect of high volume hemofiltration (HVHF) on expression of TNF-α and IL-8 mRNA in lungs of a porcine multiple organ dysfunction syndrome (MODS) model. Methods: Twenty-five pigs were randomized into model group (n=9), control group (n=6) and HVHF group (n=10). Double-hit method was used to establish the porcine MODS model. Pigs were treated with HVHF for 3 d in HVHF group and left without treatment in the control group. Organ-function related bio-chemical indices were determined. Real-time quantitative PCR was used to determine the mRNA expression of TNF-α and IL-8. Results: Indices of organ function improved to various extents in HVHF group compared with those in model group. The morbidity and mortality of MODS pigs were much lower than those in model group (20.0% vs 88.9% and 20.0% vs 77.8%, respectively,P<0.05). In HVHF group, the mRNA expression of TNF-α ([0.104 2±0.015 8] vs [0.123 5±0.018 8],P<0.01) and IL-8 ([0.145 8±0.019 9] vs [0.199 1± 0.051 9],P<0.05) was decreased significantly compared with those in model group.Conclusion: HVHF can inhibit trauma-induced over-expression of TNF-α and IL-8 mRNA in lungs when used during the early period of trauma, and attenuate pulmonary injury and dysfunction.
