几丁质管防治神经瘤形成的实验研究与临床应用 被引量:4

Chitin Chembers Bridging both Proximal Ends of Two Nerves for Prevention and Treatment of Painful Neuroma

摘要 采用几丁质生物材料制成的管道套接兔正中神经和尺神经两近端防治神经瘤形成。通过大体、光镜和电镜观察,结果证明:几丁质具有显著的抑制神经瘤形成的作用。在实验研究的基础上,笔者用几丁质管套接治疗4例残肢(指)由神经瘤引起的顽固性疼痛和1例截肢的同时套接两近端神经防治神经瘤。共14根神经,通过6个月以上随访,4例顽固性疼痛完全消失,1例未发生神经瘤样疼痛。 Management of the painful neuroma, particularly the recurrent meuroma is a difficult problem. There are many methods for its therapy, but the effect is not enough optimal. We introduce a new method and material used, by which good results were shown in the three experimental groups and brought it into clinical use. Three experimental patterns of chitin chamber bridged both proximals of two nerves, then nerve suture and single cutting were performed in the median and ulnar nerves of the rabbit right arm, observed by the naked eyes, light and electron microscope at 4, 8, 12 th week post operation. The nerve fibers of the regenerated nerve loop arranged regularly, The polarity was consistant, and Schwan cell body was intact at 12 th week after operation. The experiment offered a ground for clinical appliction of this method. The author treated 5 patients, and all cases were followed up for 6-8 months . The reults were optimal with the pain lost.
