用弹力裤治疗早期先天性髋脱位

Thc Use of a Pair of Self Self-designed ElasticTrousers for the Treatment of Congenital Dislocationof Hip in Infants

根据自然复位、弹性固定、有限制动的原理，作者自行研制一种治疗早期先天性髋脱位的弹力裤新方法。自１９８７年１１月～１９９０年１月用该方法治疗１８个月以内患儿６２例，８１髋，获１～３年随访５９例，７７髋，优良率达９６．１％，无并发症。该衣裤用普通布料制做，其厚度依据季节不同而异，穿戴舒适，可随时进行更换清洗，其弹力固定带的厚及长度可根据患儿年龄大小、肥胖程度、脱位情况及治疗的不同阶段任意进行调整，既可将其固定在Ｌｏｒｅｎｚ氏的蛙式位，又可允许其髋关节在Ｒａｍｓｅｙ氏的安全范围带内活动，使脱位的髋关节在活动的过程中逐渐复位，不需使用任何麻醉及手法整复，消除了复位时的暴力，可有效地避免股骨头缺血性坏死等并发症。有限的自主活动可使头臼间相互刺激，共同发育和再塑，有利于髋关节解剖及生理功能的恢复。

Abstract From November 1987 to January 1990, theauthors had treated 62 cases of congenital dislocation ofthe hip with a pair of seif-designed elastic trousers,which were made of common cloth of various thicknesssuitable for the changing seasons , The trousers can beworm comfortably and also can be washed and changedas desired.The thickness, length of the elastic bandscan be adjusted according to the children’s age, typesof the dislocation during the different stages of thetreatment at the out patient departmint, The patientcan be fixed in Lorenz’s frog position and the hips wereallowed to move within Ramsey’s safety range. Thedislocated hip joint can be reduced gradually during themovement,The reduction was accomplished withoutthe use of anesthesia or manual violence and ischemicnecrosis of the femoral head can be avoided.The 62cases ( 81 hips)int this group were all under 18 monthsof age and were followed-up for 1～3 years. Theresults in 59 cases (77 hips) were excellent (96.1%).
