Delphi法确立临床护理专家的资格认证方法

Study on the Establishment of the Evaluation Index System of Clinical Nurse Specialists

目的确立临床护理专家的资格认证方法,以实现临床护理专家规范化、系统化的认证考核,为相关职能部门提供参考依据。方法采用Delphi法对34名专家进行2轮问卷调查,问卷最后设置开放性问题,进一步了解专家对表中未提及内容的意见和建议。结果专家调查第1轮和第2轮的有效回收率分别为80.0%和88.2%,充分反映了专家们对本研究的关心和支持。本课题选择的专家涉及护理管理、临床护理、护理教育3个领域,所选专家具有较好的学科代表性和地区代表性,专家的权威程度0.8以上占到70.0%。第1轮总体指标协调系数为0.307,第2轮总体指标协调系数为0.578(P均<0.001),协调系数具有显著性,说明专家意见协调性好,预测结果可取。结论临床护理专家的培训基地应由医科大学护理院系或护理硕士培养点与专科特色医院进行联合培养;临床护理专家的认证机构由卫生行政管理部门授权医科大学护理院系进行认证组织工作;临床护理专家的认证方式采取个人申请、单位推荐、认证机构组织专家进行综合评定;临床护理专家的认证考核内容主要包括个人基本情况评价、专业态度、专业知识技能考核、临床角色作用评价4个主要部分共计27项指标。

Objective To establish the evaluation index system of clinical nursing specialist, and to implement clinical nursing specialist certification and examination normalizing systematizing, to provide reference bases for correlating functional authority department. Methods With Delphi method, 34 clinical nursing specialists were investigated for two times. The questionnaires were designed with questions, to further understand the opinions and suggestions to the contents not mentioned in the list. Results The recovery rates of two times were 80.0% and 88.2% respectively. It reflected the care and support of the specialists to the study. All specialists, included nursing supervision, clinical nursing, nursing education, have good representativeness in subjects and area. The authority degree more than 0.8 is 70.0%. The concordance coefficient of the general index is 0.307 in the first investigation, and that of the second is 0.578 (P< 0.001), the difference is significant. This indicated that the concordant of the specialists’ opinions is good, and the forecast result is desirable. Conclusions The clinical nursing specialists should be trained by nursing college or nursing faculty or master of nursing cultivation agency in medical university combined special hospital. The certification agency of clinical nursing specialists should be granted to the nursing faculty of medical university by health administration department. The certification patterns are with individual application, unit recommendation and composite evaluation of specialists organized by the certification agency. The contents of examination mainly included four parts, the evaluation of individual basic condition, attitude of profession, specialized skill and knowledge and the clinical role action, all are twenty-seven items.
