青年突发迷走神经性晕厥发病机制的初步探讨

Young Neurally Mediated Syncope and Endothelial Function

目的探讨青年突发迷走神经性晕厥的发病机制与外周血管内皮功能的关系。方法选择经直立倾斜试验诊断为迷走神经性晕厥的36例青年患者为晕厥组,30例直立倾斜试验除外迷走神经性晕厥的患者为对照组,利用彩色多普勒超声心动图仪对两组患者进行血管内皮功能测定。结果两组患者的年龄、性别和吸烟患者的比例无明显差别,晕厥组的血管内皮依赖性舒张功能及血管内皮非依赖性舒张功能较对照组增强:(9.08±4.01)%对(6.32±3.10)%,P<0.05;(15.26±7.69)%对(11.82±6.30)%,P<0.05。结论外周血管内皮功能异常可能是青年突发迷走神经晕厥的机制之一。

Objective To determine whether endothelial function has a significant role in the pathogenesis of neurally mediated syncope. Methods Thirty-six patients with neurally mediated syncope diagnosed by tilt testing were selected as study group and 30 patients without neurally mediated syncope(excluded by same test )were selected as the control group. All patients underwent the endothelial function examination in the brachial artery using colour Doppler echocardiography .Results There were no singificant difference in age ,gender or smoking between the two groups. Flow mediated dilation and glyceryl trinitrate mediated dilation in patients with neurally mediated syncope were greater than those in controls (9.08±4.01% vs 6.32±3.10%,P<0.05; 15.26±7.69% vs 11.82±6.30%,P<0.05). Conclusion Endothelial function abnormality play an essential role in the pathogenesis of young neurally mediated syncope.
