小儿机械通气时呼吸功能参数检测的临床评估 被引量:4

摘要 目的探讨呼吸衰竭小儿机械通气中有关肺功能的撤机指标。方法分别于上呼吸机第1天和撤机时监测患儿有效动态顺应性、总阻力、气道闭合压(P0.1)及血气分析等指标。结果在肺原性呼吸衰竭患儿,28d~1岁组、~3岁组和~16岁组,有效动态顺应性、比顺应性、总阻力及P0.1在撤机时与上呼吸机第1天比较,差异均有显著性(P<0.001或P<0.05);非肺原性呼吸衰竭患儿有效动态顺应性和总阻力在各时间段比较差异无显著性,P0.1在撤机时与上呼吸机第1天比较差异均有显著性。结论在机械通气中,对肺原性呼吸衰竭的有效动态顺应性、比顺应性、P、总呼吸系统阻力的检测可用于小儿机械呼吸撤离的预测。
