腹膜种植转移与胃癌浆膜类型相关性探讨

Types of serosal involvement for gastric cancer and postoperative metastasis of peritoneum

目的探讨胃癌浆膜类型预测术后腹膜转移的临床价值。方法术中采集100例胃癌患者腹腔灌洗液,进行细胞学检测(PLC)和放射免疫法测定癌胚抗原蛋白(PCEA)水平,将胃癌浆膜类型、腹腔灌洗液检测结果以及术后随访情况进行分析。结果腱状型和弥漫多彩型PLC和PCEA以及术后腹膜转移发生率与正常型、反应型和结节型相比较,结果有显著性差异(P<0.05)。而腱状型PLC、PCEA及术后腹膜转移率与弥漫多彩型相比较无显著性差异(P>0.05)。结论胃癌腹膜类型为腱状型和弥漫多彩型极易发生术后腹膜转移。据浆膜类型预测术后腹膜转移,是一种较可靠、简便的方法。

Objective To study the type of serosal involvement for gastric cancer and postoperative peritoneal metastasis. Methods Cytologic examination and CEA level determination by radioimmunoassay were performed in peritoneal washes from 100 patients of gastric cancer,the result was analysed among the serous type of gastric cancer and PLC,and PCEA,and postoperative follow-up survey. Results The positive rates of PLC and PCEA and postoperative peritoneal metastasis of the tendonoid and colour diffused were significantly higher than the normal and reactive and nodular (P<0.05),but the positive rates of those of the tendonoid were not significantly higher than the colour diffused (P>0.05).Conclusion The intraoperative exact judge of the peritoneal types of gastric cancer could be a more sensitive and reliable predictor of peritoneal metastasis.
