肾盂癌增殖细胞核抗原表达的临床意义

Clinical significance of PCNA expression in carcinoma of renal pelvis

采用免疫组化技术检测３ｌ例肾盂癌和９例慢性肾盂炎组织中增殖细胞核抗原（ＰＣＮＡ）的表达。发现肾盂癌ＰＣＮＡ阳性细胞数明显高于炎症组，随肿瘤病理分级和分期的上升而增加，且分化差的肿瘤细胞染色增强。还发现ＰＣＮＡ阳性细胞数大于７５％的肾盂癌术后易再发膀胱肿瘤。提示ＰＣＮＡ免疫染色对肾盂良恶性病变的鉴别和肾盂癌生物学行为的评估具有重要意义。对肾盂癌ＰＣＮＡ阳性细胞数大于７５％者术后应定期复查膀胱镜，并行必要的膀胱灌注治疗。

Expression of proliferating cell nuclear antigen(PCNA)was investigated in 31 cases of carcinoma of renal pelvis and 9 cases of chronic pyelitis tissue using the immunohistochemical technique.The result showed that the proportion of PCNA positive cells was higher in tumor tissue than that in pyelitis tissue and increased with tumor grade and stage.The poor-differentiated tumor cells had a stronger immuno-reactivity than well-differentiated ones.Tumor with the PCNA positive cells more than 75 percent were predisposed to the occurrence of bladder tumor after operation for renal pelvis cancer.These patients should have cystoscopy and intravesical therapy at regular intervals.Our findings suggest that PCNA stainning might be of value in the differentiation of benign and malignant renal pelvis lesions and in the evaluation of the biological behavior of renal pelvis cancer.
