茄病镰刀菌引起坏疽性脓皮病

Pyoderma Gangrenosum Caused by Fusarium Solani

患者男，５８岁，铁渣灼伤左上睑后，于眼睑部、左颞部及鼻部迅速出现红肿、化脓、坏死、结痂、溃烂，深达皮下及肌层。皮损区直接镜检及组织病理切片均发现大量真菌菌丝，８次真菌学培养为同一菌株ＣＺ９２１３生长。该菌株经形态特征，生理测定及动物试验等真菌学研究鉴定证实为茄病镰刀菌［Ｆｕｓａｒｉｕｍ ｓｏｌａｎｉ（Ｍａｒｔ），Ｓａｃｃ］。该患者经氟康唑与５－氟胞嘧啶联合应用治愈。

A 58-year-old male patient was admitted in January 1992 for a trauma of left eyelids caused by iron slag over 17 days. On examination he exhibited erythema , swelling, festering, necrosis, crust and deep ulcer involving subcutaneous and muscular layers of his lids, left temple and nose, A large amount of fungal hyphae were found on direct microscopy and histopathological examination. Same strain, CZ9213, grew on mycological culture medium for & times. The strain was identified as Fusarium solani (Mart) Sacc. by morphological characteristics, physiological features, and animal experimentation. The patient was cured by fluconazole in combination with 5-flucytosine.
