神经原性高血压病的显微外科治疗

Microsurgical treatment of neurogenic hypertension

报告３０例神经原性高血压病的显微外科治疗，其中２４例作了左侧显微血管减压术（ＭＶＤ），术中均发现异常动脉袢压迫神经根及延髓，经１～６年随访，１８例血压恢复正常，３例只需服用少量降压药物，２例较术前服药量减少，１例失去随访；在６例右侧手术病人中可疑血管压迫４例，无血管压迫２例，这６例病人术后血压均无明显变化；本组无死亡。对手术适应证和手术治疗要点作了讨论。

Abstract Microsurgical treatment of neurogenic hypertension was reported in 30 patients and 24 of whom were treated by the left medullary microsurgical vascular decompression(MVD). During operation, arterial loops compression of the nerve root and medulla were discovered. The followed-up period were 1-6 years. The blood pressure remains normal at present in 18 patients, 3 patients on a little medicine. and 2 of them with smaller doses than before surgery. One lost to followup. Four patients of questionable compression and two patients no compression were operated on the right side. The blood pressure was unchanged after surgery. There were no mortality in this group. The indication and surgi cal approach were also discussed.
