摘要
通过门静脉高压症的脾脏厚度与门静脉系统血流动力学指标间的相关性分析发现:脾脏厚度与门静脉主干内径、脾静脉内径、脾静脉血流量及切脾后自由门静脉压(FPP)呈正相关,与门静脉主干的血流时间平均速度呈负相关,差异有显著性意义(P<0.05)或有非常显著性意义(P<0.01)。说明脾脏越大门静脉系统的高阻力、高动力血流状态越严重。切脾后FPP值较切脾前FPP值显著降低(P<0.01)。离断术后FPP值又升高,但未达切脾前FPP值水平,脾肾静脉分流术后FPP值与切脾后FPP值相比几乎无改变。说明脾切除是降低FPP的主要因素,选择含有脾切除的手术方式是治疗门静脉高压症较合理的手术方法。
The relation analysis between
splenic thickness and portal hemodynamics showed that there was a posotive relationship
betweensplenic thickness and MPVD. SVD. SVF. FPPafter splenectomy. But there was a
negative relationship between splenic thickness and MPV-TAV. The differences were
significantP<0.05)or highly significant (P<0.01).These results showed that the bigger the splenic
volume. the more serious the hyperdy-namies and that high resistance state of bloodflow of the
portal system. The value of FPPdropped markably after splenectomy, but there was an increase
in FPP value after devascular-ization. However, the FPP value after devascu-larization was
lower than before the splenecto-my,The FPP value after splenectomy did notshow any
significant change when splenic-re-nal vein shunt operation was performed Ourresults suggest
that splenectomy is the mainfactor for decrease of the FPP.Therefore,sur-gical interventions
including splenectomy forthe treatment of portal hypertension are justi- fiable.
