高颈段椎管哑铃型神经鞘瘤

Dumbbell Iike neurinoma at upper cervical vertebra:Report of 48 cases

作者对４８例高颈段哑铃型神经鞘瘤患者行肿瘤全切除并经病理证实。本病大多以颈枕阵发性疼痛，颈部活动受限，一侧或双侧腔体麻木为首发症状。行颈椎双斜位Ｘ线检查可发现椎间孔扩大，进一步行ＭＲＩ可明确诊断，推动脉造影对手术治疗大有稗益。作者还对手术中保护推动脉及呼吸功能方面的体会进行了讨论。

Abstract4 8 dumbbell like neurinoma at upper cervical ver-tebra were totally removed operatively and confirmedpathologically. Intermittent pain at neck and occiputlimitation of neck movement,and numbness or pain atone or bOth side limbs were the most common symp-toms,In the early stage,the disease was difficalt torecognize. If X-ray film of the cervical vertebra at bi-oblique position showed an enlarged intervertebral fo-ramen,the diagnosis of neurinoma should be highlyconsidered.MRI could make a definite diagnosis.Ver-tebral artery angiography is of great value,We also in-troduced experience in how to protect vertebral arteryand respiratory function during operation.
