肺癌纵隔淋巴结转移及清除的必要性

A Disscussion on Neccesity of Mediastinal Lymph Nodes Dissection-Extent and Distribution of Lymph Nodes Metastasis in Lung Cancer

１９８７年１月至１９９２年１２月为３１４例肺癌病人行根治性切除手术。术中对同侧肺门和纵隔淋巴结进行广泛清除。３１４例肺癌中，Ｎ２（纵隔淋巴结转移）１１９例，占３７．９％。作者总结了Ｎ２的范围和分布特点，以及Ｎ２和肺癌细胞类型、部位间的关系，认为从根治目的出发，肺癌手术有必要彻底清除胸腔内淋巴结。

From January 1987 to December 1992, radical pulmonary resections with extensive dissection of the unilateral hilar and all groups of the mediastinal lymph nodes were performed on 314 patients with lung cancer in our hospital.The pathological examinations showed that N2 metastasis occurred in 119 (37.9%).The author summarized the extent and characteristics of the distribution of N2metastasis and its relationship to the type and location of the lung cancer.And concludes radical surgical treatment of lung cancer should include redical pulmonary resection, and extensive and thorough dissection of intrathoracic lymph nodes.
