白内障囊外摘除术中的连续环形撕囊术

Continuous circular capsulorhexis in extracapsular cataract extraction

为了在临床上探讨连续环形撕囊术的优点、方法及并发症的预防，我们对５２只眼白内障囊外摘除及一体性后房型人工晶体植入术用连续环形撕囊术进行前囊膜破开，采用“开罐式”破囊的４０只限作为对照。术后１个月散瞳后通过房角镜观察前囊膜及人工晶体襻的固定位置。结果：术中可见连续环形撕囊组中有４８只眼获得直径约６．５～７．０ｍｍ边缘锐利的圆形撕裂孔，术后４４只眼（８４．６％）人工晶体的双襻均在囊袋内，而“开罐式”破囊组术后仅２１只眼（５２．５％）双襻在囊袋内。说明连续环形撕囊术可保持囊袋的完整性，增加人工晶体襻囊袋内固定的比例。

To investigate clinically the advantage,method and prevention of complication of contin-uous circular capsulorhexis,a surgery consisting of continuous circular capsulorhexis,extracapsular cataract extraction and one-piece PMMA intraocular lens(IOL)implantation was performed on 52 eyes.The control was “canopener”capsulotomy made in 40 eyes. The residual anterior capsule and loop fixa-tion of IOL were observed by dilatation of the pupil and gonioscopy 1 month after the operation. Re-sults: Postoperatively an anterior capsular opening of 6.5～7.0 mm in diameter with sharp edge could be seen in 48 eyes and the fixation of both loops in the capsular bag was found in 44 eyes (84.6%)in capsulorhexis group and 21 eyes(52.5%)in “canopener”capsulotomy group. It is demonstrated that continuous circular capsulorhexis could maintain the integrity of the capsular bag and increase the chance of achieving a fixation of IOL in the bag.
