原发性甲状腺功能减退甲状腺片治疗监测指标分析

An Analysis of Laboratory Results in Patients with Primary Hypothy-roidism Receiving Thyroid Tablet Treatment

本文对１２３例原发性甲状腺功能减退（甲减）患者经甲状腺片替代治疗后Ｔ３、Ｔ４、ＴＳＨ三项监测指标进行了分析，结果提示在甲状腺片治疗过程中Ｔ３反应较为敏感，而Ｔ４对甲状腺片治疗反应较慢，部分病例在治疗过量时Ｔ４仍不能正常，提示在临床甲减甲状腺片治疗中，应以观察ＴＳＨ及Ｔ３变化为主，不能追求Ｔ４正常。用免疫放射法（ＩＲＭＡ）测定的ＴＳＨ由于灵敏度明显提高，在评价甲状腺片替代治疗的效果特别是在判断替代过量方面明显优于放射免疫法（ＲＩＡ）。

The Results of serum T3,T4 and TSH estimation in l23 patients with primary hy-pothyroidism receiving thyroid tablet treatment were analysed. The data showed that TSH andT3 responses were more sensitive than T4 response to thyroid tablet replacement treatment.T4 levels were still below the normal range in part of these patients though overdoses of thyroidtablet were.It is reasonable to regard TSH and T3 as main indexes in the evaluation of theeffect of thyroid tablet treatment.TSH values obtained by IRMA, due to its high sensitivity,are superior to those by RIA in evaluating the effect of replacement treatment,especially forthe judgment of overtreatment
