C－erbB－2癌基因产物在细支气管肺泡细胞癌和细支气管肺泡细胞增生的表达

Expression of oncoprotein C-erbB-2 in bronchiolo-alveolar carcinoma and atypical bronchiolo-alveolar cell hyperplasia

本研究应用单克隆抗ｃ－ｅｒｂＢ－２抗体ＰＡｂ１对２６例细支气管肺泡细胞癌（ＢＡＣ）和一组癌旁肺组织非典型细支气管肺泡细胞增生（ＡＢＨ）进行免疫组织化学研究，显示ＢＡＣ确有ｃ－ｅｒｂＢ－２癌基因的过表达，其中ｃｌａｒａ细胞型阳性率为７０％；Ⅱ型肺泡细胞型为５０％；粘液柱状细胞型为５０％ｃ－ｅｒｂＢ－２阳性表达与肿瘤的部位、大小及有否淋巴结转移均没有关系。根据目前所获随访资料看，ｃ－ｅｒｂＢ－２表达似与病人预后无明显关系。在１０例癌旁ＡＢＨ中有１例中度ＡＢＨ呈ｃ－ｅｒｂＢ－２阳性和１例重度ＡＢＨ显示可疑阳性。此种阳性表达意义尚不清楚，考虑到相应病例的癌细胞亦呈阳性表达，可提示该增生的、表达ｃ－ｅｒｂＢ－２癌基因的ＡＢＨ细胞亦受到基因突变的影响，可能成为ＢＡＣ的癌前病变信号。

Twenty -six cases of bronchiolo -alveolar carcinoma (BAC) and ten cases of atypical bronchiolo - alveolar cell hyperplasia (ABH) in paracarcinoma were studied to observe the expression of C - erbB - 2 oncoprotein with immunohistochemical method (ABC method). Seven out of ten of Clara cell type, five out of ten of type Ⅱalveolar epithelium cell type and three out of six of mucinous columnar cell type exhibited positive staining, indicating amplification of the gene in these tumors. There is no any relationship between the overexpression of C -erbB - 2 and each of the following items: tumor location, size and lymphatic metastasis. Based on the follow - up data,it seems that there is no marked relationship between the expression of C -erbB - 2 oncogene and prognoses of patients. One out of ten of ABH close to carcinoma exhibited positive staining and another one severe case with ABH presented questionable positive staining. The significance of such positivity is unclear. It is suggested that the ABH cell also undergo mutation of gene and the oncogene expression be a sign of precancerous lesion.
