不同类型的卵巢纤维上皮瘤病理及预后

Clinicopathological observation on the varius classification Brenner's tumor ovary

选择１２例不同类型的卵巢纤维上皮瘤进行临床和病理方面的比较观察，其中良性纤维上皮瘤４例；增生性纤维上皮瘤３例；恶性纤维上皮瘤２例；移行细胞癌３例。组织学上，鉴别良恶性纤维上皮瘤的关键是判断是否有间质浸润，恶性纤维上皮瘤和移行细胞癌均有间质浸润，但后者无良性或增生性纤维上皮瘤成分。２例恶性纤维上皮瘤均有增生性纤维上皮瘤成分，支持恶性纤维上皮瘤由增生性纤维上皮瘤转化而来的观点。随访结果显示：移行细胞癌预后差。我们认为区分卵巢纤维上皮瘤的类型和分化程度对指导临床治疗和判断病人预后具有重要意义。

The clinical and pathologic features of 12 cases of various classification Brenner's tumor of ovary,including 4 benign,3 proliferative,2 malignant Brenner's tumor(MBT)and 3 ovarian transitional cell carcinomas(TCC)were compared.Histologically,it is important to recognize the stromal invasion in the differential diagnosis of benign and malignant Brenner's tumor.Though both MBT and TCC were stromal aggressivcness, MBT differed from TCC by the presence of benign and proliferative Brenner's tumor component,this supported the concept that MBT arise from proliferative Brenner's tumor,The results of follow-up inspection showed that the prognosis of TCC is poorer,which suggests that to differentiate the degree and types of Brenner's tumor are of therapeutic and prognostic significance.
