上消化管粘膜下良性肿瘤的超声内镜诊断与内镜下治疗

Upper gastrointestinal submucosal tumor: endoscopic ultrasonographic diagnosis and endoscopic therapy

内镜下治疗上消化管粘膜下肿瘤２５例。２２例采用单纯圈套切除术，２例采用肿瘤顶部粘膜切除加瘤体摘除术，１例采用分次套切加无水乙醇注射法。术后６例发生轻度出血，２例发生中度出血，内镜下高频电凝、注射药物均可止血，无其他严重并发症。术前１２例行超声内镜检查，与术后病理结果比较，超声内镜对上消化管粘膜下肿瘤的术前诊断具有较大价值。

Twenty-five patients with upper gastrointestinal tract submucosal tumor were treated endoscopically using high frequency snare.Twenty-two tumors were resected through single high frequency electroectomy. Two tumors were resected at two procedures, first making mucosoectomy and then resecting the tumors. One tumor was resected at multiple procedures combining with injection of absolute alchol. These procedures were not accompanied by serious complications such as perforation or massive GI bleeding. Slight or moderate bleeding was found in 8 cases, which was easily stopped by injection of sclerotor or electrocoagulation with an endoscope. Before operation eleven patients had endoscopic ultrasonography (EUS) examination, and as compared with postoperative histological examination, we found that EUS had a great value in preoperative diagnosis of submucosal tumor.
