摘要
本研究采用液压打击脑损伤模型,以微量透析技术采集脑细胞外液(ECF),观察大鼠脑损伤后脑ECF中乳酸含量的变化及其与脑水肿的关系。结果表明:(1)液压打击伤后,损伤区ECF中乳酸含量显著升高,其升高值与脑损伤的程度相关;(2)ECF中乳酸增加以伤后0~20min时间区段最高,伤后1h外伤组ECF中乳酸含量尚未恢复至正常水平;(3)脑外伤后ECF中乳酸含量与伤后脑水肿程度呈正相关。提示伤后ECF中乳酸含量的改变,可作为判断伤情和预后的一个重要生化指标。
The male Sprague-Dawley rats were used in the experiments. A fluid percussion device was used to produce experimental traumatic brain injury(TBI). The extracellular fluid(ECF) in the regions of injury was collected by microdialysis. The changes of lactate in the ECF following moderate and severe TBI were observed. During the experiments, the varieties of arterial gas, mean arterial blood pressure(MABP) and the water content of brain were also observed. The results showed that (1) the lactate level in the ECF following TBI was increased significantly and was related to the extent of brain injury; (2) the increase of lactate in the ECF reached peaking within 20 min after TBI, and did not decrease to normal level within 1 h after TBI; and (3) the increased lactate in the ECF was related to the extent of brain injury and brain edema positively. These findings led us to conclude that the changes of lactate in the ECF are one important biochemical factor which could be used to determine the extent of TBI and predict the prognosis.
