狼疮性肾炎肾组织细胞胞浆内异常超微结构

摘要 对24例狼疮性肾炎肾穿刺标本,着重在电镜下观察肾组织细胞胞浆内两种和内质网有关的异常超微结构。结果18例发现管网状结构(TRS),其中10例同时发现圆柱状对合池(CCC)。TRS常见于肾组织毛细血管内皮细胞,该处的TRS与疾病的活动性似无密切联系。CCC多见于间质中的单核/巨噬细胞和淋巴细胞,和疾病活动性、肾间质免疫沉着均有密切联系。本文还对CCC的形态变化及形成过程进行描述,指出TRS和CCC均由内质网膜“芽生”而来,属特征性病变,结合一定的临床或/及病理背景有辅助诊断价值,故值得重视。 An electron microscopical study of 24 renal biopsies from patients of lupus nephritis was carried out,withparticular emphasis on two endoplasmic reticulum-related structure.Tubuloreticular structure(TRS)was foundin 18 cases,and cylindrical confronting cisternae(CCC) in 10 of them simultaneously.TRS often appears in thecapillary endothelium,and does not correlated well with the activity index of lupus nephritis.CCC appears fre-quently in monocyte and lymphocyte,and correlates well with both the activity index and the amount of inter-stitial immunologic deposit.The morphogenesis and morphologic variations of CCC were also described in thispaper,and it was expounded that TRS and CCC derived from the“budding”of endoplasmic reticulum mem-brane,Both TRS and CCC are pathognomonie,though not specific changes. They may be helpful in patholog-ic diagnosis,when combined with clinical and pathologic features.
