摘要
用抗地高辛单克隆抗体(Dis-McAb)IgG及其Fab片段救治豚鼠(分别为5只和7只)严重地高辛中毒性室速、高度房宣传导阻滞等心律失常,全部获得成功。用生理盐水或无关McAbFab片段救治的对照组豚鼠(分别为5只和6只)全部死于中毒性心律失常,平均死亡时间为10.8±2.4min。抗体用量为中毒地高辛量的化学当量的1.5倍,静脉注射。Fab片段治疗组中毒性心律失常逆转时间(5.1±1.0min)明显短于IgG治疗组(12.0±2.0min),表明Dig-McAb是地高辛中毒的特效解毒剂,且Fab片段用于解毒更为优越。
For the purpose of the extensive application of immunologic treatment of digitalis intoxication, digoxinspecific monoclonal antibodies IgG and Fab fragments were tested for their efficacy in reversal of advanced, otherwise lethal digoxin toxicity in guinea-pigs. Two control groups (5 animals in one group and 6 in the other) were given nonspecific mouse Igs or normal saline, and all died 10.81±2.35 minutes after the onset of toxic arrhythmias. Twelve animals in two test groups (5 and 7) treated with digoxin-specific monoclonal antibodies IgG and Fab fragments in a dose 1.5 times the chemical equivalent of digoxin dose recovered from the established toxic arrhythmias. The mean time of full recovery from overt digoxin-induced arrhythmias (5.14 minutes) in the Fab fragments treated guinea-pigs was significantly shorter than that given intact digoxin-specific monoclonal antibodies IgG (12 minutes). It can be concluded that monoclonal antibodies and their Fab fragments are capable of reversing advanced and otherwise lethal digoxin toxic arrhythmias and that the Fab fragments have several potential advantages over the intact IgG for detoxication.
