慢性阻塞性肺病呼吸衰竭患者机械通气连接方式的改进

The change in the ways connecting to the ventilator in COPD patients with respiratory failure.

本文总结1982、1987和1992年我科呼吸监护病室收治的慢性阴塞性肺病(简称慢阻肺)呼吸衰竭资料.机械呼吸者占收治数的比例分别为41.7％、53.3%和42.9％,无明显差别(P>0.05).机械呼吸连接方式1982年以气管切并为多(8/10),1987年则大部分采用经鼻气管插管(6/8),1992年机械呼吸的9例中7例经面罩通气。存活病人中气管切开者平均住院天数163±81 d,气管插管者则为77士27 d,面罩机械通气者49士14 d,有显著差异(P值均<0.05).

During the past ten years the ways connecting the ventilator in COPD patients with respiratory failure have been changed.In 1982 most of the patients who were mechanically ventilated were tracheostomized,and in 1987 nasal intubation was the preferable way.In 1992 all but 2 cases were ventilated artificially with face mask.The mortalities in 1987 and 1992 were lower than that in 1982.The survivals who had been tracheostomized were hospitalized for 163±83 days,and those intubated nasotracheolly for 77±27 days,moreover those ventilated with face mask for 49±14 days.
