术前ERCP对腹腔镜胆囊切除术的意义

Evaliation of the use of ERCP prior to LC

９３例临床表现为单纯胆囊良性疾患的病人行术前ＥＲＣＰ检查，成功率为９３．５％。其中发现胆道变异５例（５．７％），合并胆总管结石６例（８．５％），并在术前行内镜乳头切开取石术，８８例接受腹腔镜胆囊切除术，通过对其ＥＲＣＰ影像特点与术中难易程度关系的分析，提出ＥＲＣＰ对于腹腔镜胆囊切除术前选择病例，判断术中操作的难易程度有重要的参考价值，内镜乳头切开取石与腹腔镜胆囊切除相结合是目前处理合并胆总管结石病例的安全有效的方法。

Preoprerative ERCP was performed in 93 patients whose clinic appea appea rance were simple benign gallbladder disease with the successful rate of 93. 5% Atypical bile duct anatomy was seen in 5 patients(5.7%),ERC also revealed bile duct stones in 6 cases(8. 5%)and EPT succeeded in all of them. LCwas performde in 54 cases.It is concluded from an analysis of the relationship between the ERC appearance and LC difficulty that ERCP has great help for the LC in the area of case selection and difficulty prejudgement LC associated with EPT is now a safe and simple management of the patient still with choledcholithiasis
