老年高血压性肥厚性心肌病8例报告

Hypertensive Hypertrophic Cardiomyopathy in the Elderly (Report of 8 cases)

根据Topol提出的老年高血压性肥厚性心肌病的概念报道一组（8例）具有高血压病史且有严重的左心室向心性肥厚,左室腔径缩小（EDD4.62±1.0cm,ESD2.7±0.9cm）,收缩功能指数增加（EF 69.0±20.7％,△D％40.0±14.7％）,舒张功能受损（EF slope 69.9±33.4mm/s）等特点,而不同于一般的高心病和原发性肥厚性心肌病的老年患者。患者究竟属高心病的一个特殊类型还是肥厚性心肌病患者伴有高血压,目前尚未定论。

Hypertensive hypertrophic cardiomy-opathy was diagnosed by echocardiography in 8 cases of hypertension. The patients were characterized by old age(64. 1@13. 9 yrs),a long history of hypertension (21@8 yrs),severe concentric cardiac hypertrophy, small left ventricular cavity, hyperkinetic systolic cardiac function and impaired diastolic cardiac function. These characteristics were distinctive from those of hypertensive heart disease and idiopathic car-diomyopathy in the elderly.
