摘要
在多条血管束植入治疗成人股骨头缺血性坏死的基础上，针对股骨头严重塌陷，作者运用了多条血管束加带缝匠肌骨瓣植入的新方法，自１９９３年５月～１９９５年４月共治疗５３例５５髋。按改良Ｆｉｃａｔ分期，Ⅲ＿Ａ，期３髋，Ⅲ＿Ｂ期２０髋，Ⅲ＿ｃ期１２髋，Ⅳ＿Ｂ期６髋，Ⅳ＿ｃ期１４髋，中度塌陷８髋，严重塌陷４７髋。２５髋随访半年～２年，平均１年。临床及Ｘ线评价参考第一次全国骨坏死会议制定的疗效标准，疼痛与关节活动度均较术前明显改善。术后Ｘ片优良率９２％，无一例塌陷较术前加重。
On the basis of vascular bundle transplantation for the treatment of avascular necrosis offemoral head in adults, we designed a new method which transplanted both vasular bundleand the bone with M.sartorius to treat avascular necrosis of femoral head with severecollapse. From May 1993 to April 1995 we had treated 53 cases,55 hips. According to im-proved Ficat stating system there were 3 stage Ⅲ_A, 20 stage Ⅲ_B, 12 stageⅢ_C, 6 stage Ⅳ_B,14 stage Ⅳ_C of the 55 hips in this series. The degree of head collapse was intermediate in 8cases and severe in 47 cases. 25 cases were followed up for 6-24 months(with an average ofone year).The clinical and x-ray assessent was based on the evaluation of curative effectswhich formulated in the First National Meeting of Necrosis Bone. After operation, the painwas relieved and hip motion was less limit than before. The total repair rate in X-ray was92%. There was no further collapse of the femoral head.
