摘要
观察了37例心梗患者和28例正常对照组的^(99m)TC-MIBI首次通过法左室造影(FPRNA)检测左室射血分数(LVEF)及心肌断层显像加作短轴面左室腔-心肌比值(C/W),旨在了解一次注药进行两种分析对于缺血性心肌病变的心肌灌注和心功能变化的临床实用价值,并结合超声心动图(UCG)的LVEF和左室内径对比观察.结果示:(1)心梗组的FPRNA的LVEF,C/W均显著低于对照组LVEF,C/W(p<0.05~0.01).(2)心梗组中FPRNA的LVEF与UCG的IVEF相关显著(r=0.62,P<0.01),与左室内径呈负相关(r=-0.63,P<0.01).与梗塞节段数是显著负相关(r=-0.79,P<0.001).(3)心梗组中C/W与FPRNA的LVEF呈显著正相关(r=0.69,P<0.001);与左室内径呈显著负相关(r=-0.51,P<0.05);与患者梗塞节段的数量呈显著负相关(r=-0.76,P<0.001).(4)C/W对于心功能不全的检出敏感性92%,特异性50%,准确性75%.
Imagings in 37 cases with IM and 28 subjects in normal control were made us-ing 99mTC-MIBI first-pass radionuclide angiocardiography (FPRNA) and radionuclide my-ocardial tomography. We observed the left ventricular ejection fraction (LVEF) and cavity-to-myocardial count ratio (C/W) in the two groups. The results showed that the LVEF of FPRNA and C/W in MI group were significantly lower than that in normal group (P<0.05) There were positive correlations between LVEF of FPRNA and LVEF of UCG. Also,there were both negative correlations between LVEF and ventricular size as well as infarction segmentals ; there were positive correlations between LVEF and C/W in MI group. C/W for studying LVEF ventricular function yielded sensitivity of 92% and specificity of 50% and ac-curacy of 75 %.
