头颈部腺样囊性癌76例临床分析

A Clinical Analysis of 76 Cases of AdenoidCystcarcinoma of the Head and Neck

本文报告１９８４年２月～１９９２年２月收治经病理证实的腺样囊性癌７６例。部位分别为上跨、颌下腺、腮腺、鼻腔、上颌窦、舌下腺、舌、唇。７３例采用手术治疗或以手术为主的综合治疗，３例采用单纯放疗或化疗。４４例术后复发，２１例出现肺、骨等部位远处转移。全组３、５、１０年生存率分别为７５．５％、６０．３％、２７．１％。作者认为预后与临床分期、肿瘤部位、治疗方法有关。治疗失败的主要原因是局部复发，其次为远处转移。对原发灶采取尽可能大范围手术切除，甚至不惜牺牲一些器官，术后常规放疗，可望提高疗效。

Seventy-six cases of salivary adenoid cystcarcinoma were pathologically identified andadmitted into this department from February 1984.to February 1992. The'tumor originatedfrom soft and hard palate,submaxillary gland,nasal cavity,maxillary sinus,sublingual gland,tongue and lip4 73 cases were treated by operation or operation combined with other methods.3 cases were treated by just chemotherapy or radiotherapy.Local recurrence occured in44 cases after operation. Distant metastasis was discovered in 21 cases. The survival rate of3、5、10 years was 75.5%,60. 3%, 27.1 % respectively.The authors consider that the prognosis be related to clinical stage,the region of the tumor and the methods of therapy. The maincause of failure was local recurrence,another being distant metastasis.The authors believethat the safety margin around the resected specimen should be wide enough,even sparing nosacrificing some organs lradiotheraty ought to be routine after operation,so there is hope oftherapy result improved'
