摘要
目的探讨脑电图(EEG)监测对新生儿缺氧缺血性脑病(HIE)的诊断及预后评估价值。方法总结28例HIE患儿EEG监测结果与临床诊断及预后评估关系。结果EEG异常率78.57%,轻、中、重度HIE患儿EEG异常率分别为58.33%、87.50%、100%。轻度异常者后遗症发生率为37.50%,中、重度异常者为50%。24例随访患儿中出现癫痫4例,脑瘫3例,IQ低下2例。结论EEG能正确反映HIE患儿的脑功能状态,可作为HIE的分度及脑损伤的一个重要监测指标,对于HIE的诊断及预后评价具有重要意义。
Objective To investigate the value of electroencephalogram (EEG) monitoring in diagnosis and prognosis for neonates with hypoxic- ischemic encephalopathy (HIE). Methods EEG monitoring was perfomled in 28 neonates with HIE.The relationship between EEG results and clinical diagnosis was evaluated, meanwhile, the prognostic value of EEG for patients with HIE was evaluated. Results The total abnormality rate of EEG was 78.57 % (22/28), in which the mild, moderate and severe abnormality rate of EEG was 58.33%, 87.50% and 100% respectively. The incidence rates of clinical sequelae were 50 % in patients with moderate and severe abnormal EEG and 37.50 % in patients with mild abnormal EEG. There were 24 patients who were followed up, in which 4 were epileptic, 3 were cerebral palsy and 2 were of lower IQ. Conclusions EEG may reflect correctly the state of patients' brain function, and it can be concidered as an important monitoring index. So EEG plays an important role in the diagnosis and the prognosis for neonates with HIE.
