改良青光眼小梁切除滤过术及其临床应用

Clinical application of modified glaucma trabeculetomy

目的对青光眼小梁切除滤过术进行改良,以降低青光眼小梁切除滤过术后房水引流不畅及斑痕形成发生率。方法利用自体巩膜条作为引流物,在常规小梁切除术后的巩膜瓣后,再做一板层巩膜条,植入巩膜瓣下,引流房水。结果治疗38例(50只眼),术后早期前房形成良好,眼压正常;随访2年,1只眼滤过泡消失(手术失败),49只眼维持功能滤过泡,眼压正常。结论自体巩膜作为引流物能长期保持滤过功能及防止并发症。

Objective：To prevent inadequate drainage of aqueous hunor and the postoperative cicatrice after glaucoma filtering surgery. Methods：Turning scleral flap of selfbody was performed over drainage surgery.Results：The anterior chambers in 50 eyes were well-formed by postoperative observation and the intraocular pressures were normal. 2 year later, only 1 filtration bleb disappeared, filtration blebs in anther 49 eyes maintained functional conjunctival blebs of filtration ,and intraocular pressures remained normal. Conclusion ：This study suggests that the turning scleral flap of selfbody over drainage surgery is helpful to maintain filtering function for a long time and to prevent complcation after filtering surgery.
