转换酶抑制剂对急性心肌梗塞并发心力衰竭的血液动力学影响

摘要 本文以52例急性心肌梗塞(AMI)患者,随机分为依那普利治疗组和对照组,观察用依那普利(Enalapril)前后血液动力学变化,结果表明,用药后30分,血液动力学各参数即出现显著性变化(P<0.01)。心输出量(CO)增加11%。总外周阻力(SVR)下降15%,总肺阻力(TPR)下降21.4%。于药后6h 作用未消失。提示整体条件下,依那普利对心功能的改善,主要通过影响负荷状态,增加心肌灌注,增加心输出量,减轻充血性心衰和休克的程度,以产生有益的血液动力学效应。 52 patients with acute myocardium infarction were divided at random into enalapril group and control group.The changes of hemodynamics was observed before and after enapril was used.The results showed that each index of the hemodynamics was changed immediatly,30 min- utes after the drug was used.The change was significant between the two groups(P<0.01).The cardiac output(CO)increased 11%,the total surrounding vessle resistance(SVR)decreased 15% and the total pulmonary resistance(TPR)decreased 21.4%.The action of the drug lasted 6 hours.It shows that generally the cardiac function is improved with enalapril mainly through the impact on the louding stute of heart,the increase in cardial blood perfusion the increase in cardiac output and decrease in cardiac failure and shock,producing the lemeficial hemodynamics effect of.
