肺癌脑转移22例全脑放疗疗效分析

Experience of radiotherapy to treat 22 patients with brain metastases of lung cancer

目的 总结分析肺癌脑转移的临床表现、治疗经过及预后。方法 回顾分析2 2例肺癌脑转移临床资料,所有病例行全脑放疗DT2 8～5 0Gy ,1.8～2 .0Gy/次,2～4 .5周完成。结果 全脑照射后神经系统表现及症状、体征改善15例,中位生存期6个月。复查CT显示病变消失和缩小分别为1例、6例。12例死于脑部病变进展,10例因全身病变广泛伴全身衰竭而死亡。结论 肺癌脑转移行全脑放射治疗,对减轻患者痛苦、改善生存质量、延长生存期有重要意义。

Objective To generalize and analyze the clinical manifestations, course of treatment and prognosis of patients with lung cancer metastasized to brain. Methods The clinical data 22 lung cancer cases with metastases to the brain were reviewed for pathologic confirmation and that was obtained in 10 and the rest 12 were diagnosed clinically. 13 patients had symptoms referable to the brain. All cases were undertaken whole brain radiotherapy under the given dosage of 28-50 Gy, 1.8-2.0Gy/f. Results Improvement in nervous and general symptoms and signs was observed in 15/22 patients. Complete resolution of the brain as shown by CT was observed in 1 and partial resolution in 6. The median survival of the irradiated patients was 6 months. 13 died from brain extensive metastases and general cachexia. Conclusion Irradiation to the brain is able to relieve symptoms and prolong survival in lung cancer patients with metastases to brain.
