原发性胃恶性淋巴瘤诊断和外科治疗

Diagnosis and surgical treatment of primary gastric malignant lymphoma

目的 探讨原发性胃恶性淋巴瘤(PGML)的诊断及外科治疗。方法 回顾性分析我院外科1992～2 0 0 2年收治2 2例PGML临床资料。结果 本组病例主要症状为腹部疼痛不适、消化道出血和腹部肿块,均手术治疗,手术切除率为90 .9% ,总5年生存率5 1%。结论 PGML发病率较低,临床表现、X线钡餐及纤维胃镜检查无特异性,术前极易误诊,免疫组化检查可提高确诊率,外科治疗是主要方法。

Objective To study the diagnosis and treatment of primary gastric malignant lymphoma （PGML）.Methods The data of 22 cases of hospitalized patients with PGML from 1992 to 2002 were examined in a retrospective study. Results The results suggested that the major symptoms in 22 cases were abdominal pain aceornpanied by discomfort,occurrence of gastrointestinal hemorrhage and formation of abdominal mass. All cases were undergone surgical procedures and the resection rate was 90.9 %. Total 5-year survival after surgery was 51%. Conclusion The incidence of PGML is low, but it is difficult to differentiate the sign through clinical manifestations, gastrointestinal barium X-ray or gastroscopy between gastric cancer and gastric ulcer . This disease is always misjudged before operation, fortunately, confirmed diagnosis can rely on immunohistochemical findings, but surgical intervention remains predominate.
